To renew your license, please complete the required Oregon Health Authority healthcare workforce survey.

  • When you have completed the workforce survey, please return to the Oregon State Board of Nursing nurse license portal to complete your renewal.
  • It may take up to 90 minutes for your OSBN renewal to be accessible after the survey has been completed due to OHA processing.
  • This workforce survey is designed by the Oregon Health Authority and is required for all healthcare licensees per ORS 676.410.
  • If you have any difficulty completing the survey, please contact OHA Health Care Workforce Reporting at 971-283-8792.

Enter the last 4 of your SSN, birthdate and full license number (prior to 10/2022, license numbers contain numbers and letters), and you will receive an individualized link to the survey. If you need to look up your license number, please use the OSBN verification system: License Lookup.
